According to school district policy, in order to volunteer as a chaperone, an individual must complete a "Volunteer Information Form and Waiver of Liability" and submit it to the school district for approval.
Individuals willing to chaperone band activities should download, complete, and submit these forms (to Mr. Fulkerson at the high school) as soon as possible so that the bands have a pool of individuals from which to choose chaperones.
If you believe you submitted a "Volunteer Information Form and Waiver of Liability" on or before the date indicated in the Chaperones section of the Macomb Band Boosters Committees page but your name does not appear in the list, please contact Mr. Fulkerson by e-mail or phone, (309) 837-2331.
